and you confirm going...

Please pay a deposit of
RM1500
before the end of
Monday 22nd August 2011.

Please read this simple instructions:-

1. Please read and understand the terms, condition, itinerary and the brief.

2. Please pay a deposit of RM1500 to acc no. 256-002-000246-6 (AMBANK) PUAN NURUL ANIDA BINTI MOHAMAD before the end of Monday 22^nd August 2011.

3. Please fill in your information in the personal information form from https://docs.google.com/leaf?id=0B3873dsozMJON2M5ZTM4YzctMTlmZS00M2YzLWJkNGMtMjk3NjkwMGEwOTQ1&sort=name&layout=list&num=50

4. Email one copy to delliya_zain@yahoo.co.uk and cc to nid.mohd@gmail.com.

5. Print out one copy of the personal information form and sign.

6. Attached the bank slip to the printed personal information form.

7. Please include a photo copy of the first page of your passport

8. Please include a photocopy of your IC.

9. Please print the parent’s approval form and make sure that they read and sign it.

10. Please submit all of the above on Tuesday 23^rd August 2011 to Ms Anida at Massa Block E level 5 or Ms Delliya Block C level 9.

11. Make one copy of the bank slip for yourself.
